The most difficult game I have played on the DS so far has been "Age of Empires". Maybe it's just me, but even the easy missions stump me.
I can keep tight, effective formations. I can complete campaigns in the time limit. But I cannot ever do both, no matter how hard I work at it. I certainly hope I am not the only one stumped by this game (which I love, don't get me wrong).

I've only played a half dozen or so DS games but out of those, Metroid Hunters was probably the hardest. Believe me, the others weren't hard at all.

Trauma Center. It's a fun game but when you've tried the same operation about 100 times it's time to admit the game is too hard for you. Etrian Odyssey I put under the solid good time category more than hard though.
