I haven't played that many games on my DS yet, but Etrian 1 is pretty much up there as one of the toughest. I cannot even count how many times I trekked down 5 floors from a waypoint only to fall to the boss (and then, foolishly, repeating this over and over rather than grinding).

Definitely Still Alive. Even though it's homebrew, the game is completely unforgiving and by the last few stages I'm ready to have an aneurysm trying to keep my little dude steady as I jump from platform to platform. Seriously, it goes without saying that Contra is hard, but Still Alive was a homebrew addiction of extreme difficulty I didn't see coming.

I don't know that I've played any especially challenging games on my DS, but I guess Hotel Dusk was the "hardest", I always managed to get myself kicked out of the Hotel.
